Medicare Therapeutic Shoe Program
Qualifying diabetic patients are eligible for 1 pair of extra depth shoes and 3 pairs of molded diabetic inserts. Diabetic inserts are specially designed to provide total contact plantar pressure to the diabetic patient to prevent skin breakdown including ulcers and callouses. The extra depth diabetic shoes are designed with either dual or triple depth to properly accommodate diabetic inserts.
Due to new Medicare regulations the healthcare team is required to have the following completed documents:

- Dispensing RX from a referring physician
- A Diabetic Verification Form completed by the physician that treats the patient for Diabetes
- A copy of the patient's last visit notes from the physician who treats the patients for Diabetes within the past six months. Clinical notes must state the patient's need for diabetic footwear.
All of the above documents must be received and reviewed by our office before shoes and inserts can be ordered. This change is the results of new regulations mandated by the Medicare program.
